Please print and complete the form below. Fill in your details, tear the Point Cards off the packaging and paste the Point Cards onto the form. Then mail it to the address below.

This rewards program applies to Sagami Xtreme products with the Sagami logo printed on the packaging only. Other products will not be accepted.
If the points cannot meet the minimum requirement, the redemption will be considered invalid and the points will be voided.
If the points remaining after redemption cannot meet the minimum requirement, the point will automatically be voided.
Please state clearly on the form the rewards you wish to redeem (the rewards worth 14 points will be picked and sent randomly), we will try to arrange it according to the redemption situation. However, the number of gifts is limited, while stocks last without prior notice.
If there is no indication of rewards wished, we will randomly pick and send the rewards based on the points without prior notice.
Rewards will usually be sent out within a period of 7 to 14 working days upon receipt of the form. If you have not received any rewards after a month, please get in touch with our customer service team via the below means.
